Partnership Plans

How the Partnership Plans work:
Dr. Hassell offers 4 retainer (or concierge) Partnership Plans to accommodate different levels of health care needs, which can be discussed when you schedule a consultation.

The partnerships include:
▪ Same or next day office or telephone appointments during business hours:
    1. Essential Care, typically up to 5
        per year
    2. Primary Care, typically up to 12
        per year
    3. Partnership Plus, typically up to
        24 per year
    4. Custom Care (for patients who
         have special requirements)

▪ Physician advice 24 hours a day, 7 days per week by calling our office

▪ Other services provided in our office such as EKG, lab specimen collection, injection administration, cryotherapy, ear cleaning, and 24-hour Ambulatory Blood Pressure Monitoring

▪ Oversight visits when in-patient at Providence St. Vincent Medical Center

▪ Home visit during business hours (within 10 miles of the office) when medically necessary:
    1. Essential Care, home visits are not
        included
    2. Primary Care, typically up to 1
    3. Partnership Plus, typically up to 2
